When the form is prepared for submission to the user's browser, the programm goes through a loop for each field and replaces the place holders set in Field Loop with the corresponding contents.

Header contains the html code which is printed before the first field.

Footer contains the html code which is printed after the last field.

By default the form comes with a table layout with 3 columns. You can also choose to create the form without a table, but with <div> instead.
